ENROLLMENT
Health and Welfare and Pension Benefits are determined through collective bargaining between your employer and United Food and Commercial Workers Local Union 27, 1358 or 1360. The collective bargaining results in a Collective Bargaining Agreement (CBA)/Contract. The CBA/Contract provides details as to the benefits to be provided by the Fund.
The Summary Plan Description (SPD) describes the benefits available for each plan of benefits. If you have any questions regarding the plan of benefits to which you are entitled, contact the Enrollment/Eligibility Department at the Fund office at 1-800-228-7484. Fund Enrollment form.
Changes in dependents, marital status, address, etc. should be reported to the Enrollment Department. Please don't assume that the change is forwarded to the Fund office when you notify your employer and local union. This does not always happen. Changes should be directed to the Enrollment/Eligibility Department to ensure that we have all current information on file so that you will receive all important information concerning your benefits.
