The United Food and Commercial Workers Union and Participating Food Industry Employer Tri-State Pension Plan (the "Plan") provides pension benefits payable to eligible participants who retire because of age or disability. These benefits are provided, without cost to the participant, as a result of collective bargaining between participating Employers, such as Acme Markets, Inc., SuperFresh Food Markets, Shop 'n Bag, Shop Rite, and Thriftway, and United Food and Commercial Workers Local Unions 27, 1358, 1360, and 1776. The Summary Plan Description and Benefit Booklet (SPD) is provided to all eligible participants. The SPD describes the benefits available under the Plan and how to apply for these benefits. A Statement of Benefit and Vesting credits, which provides a written estimate of benefit and vesting service under the Plan, is also provided to all eligible participants. If you have any questions regarding the Plan, wish to determine if you are an eligible participant of the Plan, or to request a SPD or an updated Statement of Benefit and Vesting Credits, please contact the Fund Office at (856) 662-8383 or 1-800-228-7484.

Q: Can I split my payment into two accounts or two banks?
A: No. We require that the net amount be deposited into a single account at a bank. You have the option of selecting either a checking or a savings account.

Q: Will I get a receipt of my deposit?
A: No. This deposit will appear on your regular bank statement. Also, some banks allow you to verify deposits made to your account via an automated telephone system.

Q: What if I change my bank and/or account number?
A: You must notify us in writing immediately of your new financial institution and/or account information. Upon receipt of the new information it will take approximately 30 days to make this change.

Q: What if I change my home address?
A: You must notify us in writing immediately of your new home address. This will enable us to forward any mailings (i.e. 1099R Form) to you correctly.
