When You Don't Know Where to Turn

Dear Member: Today's rapid pace of living, changes in society, and the impact of high technology have made stress a very common concern to most people. Stress can often cause health disorders, family disruptions, unsatisfactory job performance, and other personal, behavioral, health, or work-related problems. Your E-MAP program is designed to help you and your family deal with difficult situations that cause stress and can result in challenges to your mental health and well-being. The E-MAP program provides access to a various array of behavioral health services. The program is designed to make it as easy as possible for you to get help with personal and work-related problems. Advice given to anyone who uses the program is confidential within the limits of the law. We care about all Fund Participants covered by our Health Plan and for that reason we urge you to read this booklet carefully and share it with your family. WHAT IS THE EMPLOYEE-MEMBER ASSISTANCE PROGRAM (E-MAP)? · E-MAP is a voluntary
informational/referral service. · E-MAP is a source of assistance for personal
and work-related problems such as marital and family, child or adolescent, drug and alcohol abuse, and other related emotional problems. |
WHEN CAN I GET HELP?Participants can request help at any given time, but statistics show that by acting promptly the chances of solving your problem are increased substantially. · A participant may contact the Tri-State Health and Welfare Fund Managed Care Department directly; or · The employer, business agent, or steward may

HOW DOES IT WORK?· In order to access your E-MAP, call 1-800-228-7484, Prompt 3 to reach the Tri-State Health and Welfare Fund Managed Care Department. · Speak with a certified professional at the Fund, and obtain information regarding assessment and/or treatment choices. · A Case Manager remains available to you throughout the treatment process. |
HOW IS PRIVACY PROTECTED?· No information regarding a personal problem is given to anyone without the written permission of the person receiving the help, unless compelled to do so by a court order or subpoena. |
WHO PAYS?· The initial visits which are used for assessment, evaluation, and information purposes are free to participants. If treatment is agreed to beyond the initial visit(s), these services may be covered under the Plan's regular schedule of benefits. |

REMINDER All non-emergency treatment, either outpatient or inpatient care, must be pre-approved through the Tri-State Health and Welfare Fund Office. CALL 1-800-228-7484 MANAGED CARE DEPARTMENT Emergency care should never be delayed! Notification/Approval must be obtained from the Fund Office within 48 hours following treatment. |

REMEMBER
ACT EARLY. Don't wait. Problems rarely go away by themselves. Self-referral is encouraged and the earlier the better. The success rate is directly affected by how soon you get help. SIZE. Don't think your problem has to be large or complex. Contacting the E-MAP for an evaluation is wise. It could make a very big difference in your health and well-being. RISK. Your job is not jeopardized in any way by using the E-MAP. In fact, it can only be enhanced. CONVENIENCE. The appointment is scheduled to meet your needs. Appointments can be made after or before work. GUIDANCE. The E-MAP professional will help you map out a course of action. PRIVACY. Confidentiality is absolutely assured, unless compelled by law. |
